The Envy IOL uses extended depth of focus technology to deliver a continuous, natural range of vision — distance, intermediate, and functional near — without the hard jumps between focal points that older multifocal designs produce.
Envy Intraocular Lens
The Envy uses ActivSync Optic Technology — a lens design that dynamically adjusts how light is distributed across your retina, giving you clear vision across a continuous range of distances. Strong distance and intermediate vision with functional near. Fewer halos and less glare than traditional trifocal designs.

Standard Monofocal — Still a great option
Clear vision at one distance — usually far. You'll need reading glasses for close-up work. This is what Medicare and most insurance plans cover fully. If you don't mind glasses for reading, this is a perfectly good choice — and Dr. Malitz will tell you so honestly at your consultation.
Why one lens — not a menu
Most practices list five or six premium lenses and let the patient decide. Dr. Malitz takes a different approach.

Mastery over variety
When a surgeon commits to one premium lens, they become exceptional with it — patient selection, surgical technique, and postoperative management are all optimized for that single platform. The result is more predictable outcomes and fewer surprises than rotating between three or four lenses.

The real decision isn't which premium lens. It's whether you want a premium lens at all. Here's what each path looks like.
There's no wrong answer here
If you're comfortable wearing reading glasses and want excellent distance vision at no extra cost, the standard monofocal is a great choice. If reducing your dependence on glasses at every distance is important to your daily life, the Envy is the best tool we have for that. Dr. Malitz will recommend honestly — including telling you if a premium lens isn't right for your eyes.
Your brain needs time to adjust
The Envy lens works differently than your natural lens. There's an adjustment period — it's normal, and it's temporary.
Neuroadaptation
Your brain learned to process vision through your old lens over decades. The Envy sends a different signal — a wider range of focus than you've had in years. Most patients adapt within 3–6 months. During this time, vision continues to sharpen as your brain recalibrates.
Night vision improves
Some patients notice halos around lights at night in the first weeks — this is a normal optical property of the extended depth of focus design. The Envy's halo profile is lower than traditional trifocal lenses. For most patients, any initial halos fade significantly within the first few months.
Near vision sharpens
Reading vision often takes the longest to fully develop after the Envy. You may find yourself reaching for readers in the first weeks — that's normal. By 3–6 months, most patients achieve the functional near vision the lens was designed to deliver.
Your lens lives in the desert. We factor that in.
Lens performance in Las Vegas isn't the same as in Seattle. Light conditions, UV exposure, and dry air all affect which IOL performs best — and how we prepare your eyes for surgery.
Bright lights, low disturbance
The Strip, casino floors, and desert sunlight are a demanding visual environment. The Envy's low halo profile and high contrast optics were a key factor in Dr. Malitz's selection — this lens performs well in the conditions his patients actually live in.
UV protection built in
The Envy provides permanent UV filtering — critical for desert patients exposed to high-altitude, high-UV sunlight year-round. We still recommend sunglasses for comfort and glare, but your lens is always protecting your retina.
Dry eye and lens performance
Dry eyes can affect how well any premium IOL performs. In Las Vegas's low humidity, we evaluate and treat dry eye before surgery — because the Envy's visual range depends on a healthy, stable tear film.
How premium lens pricing works
Standard cataract surgery is covered by insurance. The Envy is an elective upgrade — your base surgery is still covered, and you pay the difference for the advanced lens.
What insurance covers
Medicare and most insurance plans cover the full cost of standard cataract surgery with a monofocal lens — surgeon fee, facility fee, and follow-ups. Your out-of-pocket cost is typically $0 or a small co-pay.
The Envy upgrade
If you choose the Envy, insurance still covers the base surgery. The premium IOL upgrade fee is an additional out-of-pocket cost that cannot be billed to insurance. We provide a written estimate at your consultation — no surprises.
